General Application Questions

Do you fund outside of Illinois?
Yes, we fund outside Illinois.

Do you fund outside of the United States?
We consider funding in countries where a board member has direct knowledge of Non-U.S. applicants.

How many grant cycles do you have per year?
We have one grant cycle. We accept proposals from October 1 to March 1 at 4:00 pm (CST) of the following year.

Is there a limit to the amount requested?
There is no limit; however, on average, our grants range from $5,000 to $25,000.

Can you help us choose a program that would best fit with the foundation preferences?
Our staff cannot help you choose a program. We ask that your request be for a program you feel has the greatest need.

Can more than one application be submitted from
our organization?

Only one application will be accepted from an organization or institution per grant year.

Does the Foundation accept multiple year requests?
The Foundation only accepts requests for one year of funding.

What day is the application due if the due date falls on a weekend?
If March 1 falls on a Saturday or Sunday the deadline is moved to 4:00 pm (CST) the following Monday.

What information do I need to open an account online?
The required information is organization name, address, phone, FEIN/EIN number as well as contact name, email, business title, mailing address and password. Click the link below to see a brief video about registration and account activation.

We have had a change in personnel and don’t know the password for our account. How do I get a new password?
Click on the link "Click Here for Grant Application" below. Begin at the Logon page. Click on Create New Account and complete your Organization Information. Organization Information will request an EIN/Tax Identification Number. Enter your Organization's number. You will receive a message warning the Tax ID# you have entered is already in our system. Send a notification to us via email as noted on the warning indicating a change in applicant name and the reason for the change so we may update our records. Continue with your registration information.

Once your email address has been confirmed you can request an application using the LOI (Letter of Inquiry) form. Complete the form including question #3 Replacement Applicant and submit your request.

I tried to set up a new account for my organization but received an error after entering the Tax ID number warning me that an account is already active and if I continue, I might create a duplicate account.
The error message indicates that an account is already active. Continue with your registration information. Send a notification to us via email as noted on the warning indicating a change in applicant name and the reason for the change so we may update our records.

Once your email address has been confirmed you can request an application using the LOI (Letter of Inquiry) form. Complete the form including question #3 Replacement Applicant and submit your request.

I have applied for a grant and would like to know the status of my request. Can I see that information online?
Yes, you can check the status of your request as it moves through our review process. Use the link below to view a video about using the online system to track your request.

Final Report Questions

I received a grant last year and need to submit a Final Report. When is the Final Report due? 
Final Report forms are due no later than December 1 following the grant year.

What if I miss the due date for my Final Report?   
Contact the Foundation office. Final Reports that are not completed by the deadline may disqualify you from future grant considerations.

I did not spend all the funds received in my grant. What should I do?
Any portion of a grant that is not used for the designated purpose within one year from receipt must be disclosed in writing to the Foundation.

How do I submit a Final Report?
Grant recipients must submit a Final Report as soon as possible after completion of their project and no later than December 1st of the year for which the funds were granted. Final Reports that are not received by the deadline may disqualify you from future grant consideration.

Final Reports are submitted online by logging into your account and completing the online Final Report Form using the link below:

After activating the link you will be taken to a Logon Page.
1. Enter your email address and password and click LOG ON
2. Your Dashboard will appear
3. Scroll to Follow-Up Forms for the grant received
4. Click Edit for the Final Report
5. Complete the Final Report
6. Click Submit

Our Final Report was not completed by the CEO/CFO/Executive Director/President of our organization. Who should sign the report?
All Final Reports must be signed by the CEO/CFO/Executive Director/President of the organization to be considered. It is a legal requirement for our process.

Final Reports are submitted online by logging into your account and completing the online Final Report Form.
